General Practitioner (GP) - Enhanced Primary Care (EPC)

In the north west of Tasmania, Stand Upright – Stay Upright! focused on enhancing the coordination of multidisciplinary interventions involving Medicare EPC items, by incorporating a falls prevention focus within the existing EPC Health Assessments protocols. This EPC item, in conjunction with a falls risk assessment tool, was used to improve early identification of individuals at risk of fall related injury and provide better-integrated multidisciplinary interventions for clients within the target group.
