Welcome to the Whose health is it anyway? Pilot Project.

This project focuses on increasing the capacity of individuals and communities to manage chronic disease.

If you have a long-term medical condition and would like to know more about:

  • Having more control over decisions about your health care
  • How to stay or become more independent
  • Using self management to reduce the impact of your disease on your daily life .

If you are a health or community worker and would like to know more about:

  • How to assist your clients to be a more active partners in their health care
  • Training and resources about chronic disease self management.
